Nominations should outline as much factual information as possible, giving details of the accomplishments of the nominees. Where possible, the claimed details of the accomplishments or service record should be supported by documentary evidence.

Nominations must be submitted on the required form and must include a 5”X7” photograph of the nominee.

Nominations will be reviewed by a “Nomination Review Committee.” If the nomination does not meet the required criteria, the nominator will be notified following the meeting. If the nomination is accepted but not chosen for that particular year for induction, the nominator will be notified and the file reviewed the following year.

All files are the property of the Hall of Fame and kept for historical purposes, unless the nomination is not accepted. Nominations are kept on file for three (3) years, and if not accepted will be returned to the nominator.

ELIGIBILITY REQUIREMENTS

TEAM:

A team must:

(a) Be based in Prince Albert and district when the competition is acclaimed and documented.

(b) Be composed of Prince Albert and district residents at the time of competition.

(c) Compete at provincial, national, international or world championships.

(d) Be composed of two (2) or more members.

The final decision regarding team eligibility is established by the Prince Albert Sports Hall of Fame board of directors on the advice of the Nomination for Induction Committee. The installation will be in the form of a suitable black and white photograph of the team
